Question#1: Do you have health and safety representatives?
If you have more than 20 employees in the workplace, then you must appoint a health and safety representative. You can get a sample appointment letter here.
Question#2: Is you ratio of health and safety representatives to employees correct?
For example, in a factory you require one health and safety representative for every 50 employees, and in other work environments you require one health and safety representative for every 100 employees in the workplace.
It's very important to note that, even if you have less than 20 employees in the workplace, and the health and safety inspector believes you should have one or more, then he has the power to tell you, in writing, to appoint more.

Question#3: Do you have a health and safety committee?
Attention! Health and Safety specialists face fines imposed by the DoL!
Recently, the Department of Labour (DoL) conducted over 190,000 impromptu health and safety inspections. It even shut down a company for failing to comply with the Occupational Health and Safety Act (OHSA).
Employers have been warned that there will be many more inspections.
Don't let them show up at your door unannounced.
By law, you MUST have a health and safety committee if you have two or more health and safety representatives.
Question#4: Are all your health and safety representative members of the health and safety committee?
At the end of the day, all health and safety representatives must automatically become members of the health and safety committee.
You DON'T have to appoint them separately from their appointment as a health and safety representative.
