To prevent an accident from happening again, get to the bottom of what caused it

 
Whenever an accident happens in your workplace, you must do an accident investigation. This investigation will help you identify the cause of the accident.
 
It'll help you do this because you can evaluate real evidence and witness testimony to discover the cause of the accident.
 
Once you identify the cause of the accident, you must discuss it with your health and safety committee. Together, you can decide on the best plan of action for dealing with whatever caused the accident.

To finally put a stop to those workplace accidents, you must take your health and safety committee's recommendations and use them to put effective safety precautions in place.
 
These safety precautions will be specifically to the cause of the accident. This is far more effective than just putting general safety measures in place and hoping they work. 
 
If you do this, you can effectively deal with those accidents that keep terrorising your workplace and ensure they never cause another problem again.
